Choosing a comfortable environment for a relative, friend, or even yourself isn’t always an easy task. Whether you are in need of short term care and rehabilitation following hospitalization or you need long term care to support and manage chronic illness or dementia, we realize this is an important decision in your life. Maintaining a strong tie to home, family and friends is of great importance to you…and to us! It is our firm belief that the quality of life is as important as the quality of care.

Who Will Care for My Loved One?

Our professional Medical Director and our in-house Nurse Practitioner work with Registered Nurses, Therapists, Dieticians, Pharmacists and other members of our interdisciplinary team to coordinate a resident’s plan of care. We have a full staff of Registered Nurses, Licensed Practical Nurses, Therapists, and Certified Nursing Assistants on duty 24 hours a day.

Skilled Nursing Services

The nursing staff will administer medications and work with the doctor and our in-house nurse practitioner to provide medical services and treatment. We encourage our residents to continue to do as much for themselves as possible; however, you can count on our team to help with bathing, dressing, feeding, and toileting when assistance is needed.

Social Services

nurse and patient smiling at the cameraThe Department of Social Services strives to foster a home-like living environment that encourages each resident to live up to their maximum potential. They provide family and resident counseling, secure spiritual guidance, coordinate shopping, and mediate grievances. Our Social Services Director functions as an integral part of our team and has an active involvement with family and resident evaluations.

Alzheimer’s and Dementia Care

If you have a loved one with Alzheimer’s disease or another type of dementia, you understand that these individuals require special care. Our team is trained to support individuals who are living with the challenges of cognitive impairment. We offer customized activities to keep residents active and engaged and to help them find joy in their days. We also provide a secure environment for the safety of individuals who may be prone to wandering.

Respite Care

For families who are caring for their loved one at home, Plantation Manor offers temporary care—for a few days or more. This provides relief for the caregiver so that he or she can enjoy a family vacation or just a rest and a little free time to take care of personal needs. It also provides a nice change of pace for their loved one who will enjoy all the social activities and amenities provided to our residents.

Hospice Care

Our specialized hospice team supports patients and families near the end of life. This team provides spiritual and emotional support as well as attending to medical needs, including making patients as comfortable and pain-free as possible. A loving environment in which all of your needs are met will enable you to more fully enjoy your precious time together.
